For participation in CYO sports during Fall, Winter or Spring seasons, submission of the physical form (scanned to AD) and acknowledgement of the Code of Conduct is required during registration.
Please print the applicable form(s) for completion by you and your medical provider. For the safety of your child, forms are due prior to any participation (including practices).
PHYSICAL FORM:

• The physical form is valid for 365 days from the date of examination.

Annual physicals must be on file with the Gesu Athletic Director. A scanned copy can be uploaded during the registration process. If the physical on file is current (within the last year), you will only need to provide the date of the physical.
This form must be submitted if your child does not meet the minimum weight requirement as determined by CYO, but still intends to play football.
This form must be submitted if you intend for coaches to consider your child for a team above his/her grade level.
The Code of Conduct is provided here for your review. You will be required to acknowledge it by checking a box during the registration process. A printed, signed form is not required.
The Grievance Policy is provided so that parents understand the proper steps to take to address any grievance they have with regard to Gesu’s athletic programs.
